GLP-1 Medications and Women’s Sexual Health: What We Know
The Part Nobody Warned Her About
She had been on the medication for about eight months. The weight loss was real, and she was grateful for it — her knees hurt less, her labs looked better, her doctor was pleased. But something else had quietly shifted, something she didn’t have a clean way to bring up at her next appointment. She wasn’t really interested in sex anymore. Not in the urgent, inconvenient way she used to be. It felt like a door had closed somewhere, softly, without announcement.
She wondered if she was imagining it. She wondered if it was stress, or age, or the long tail of a difficult year. She didn’t wonder if it was the medication — because no one had told her that was a possibility.
That gap in information is exactly what this piece is for.
GLP-1 receptor agonists — medications like semaglutide and tirzepatide, prescribed for type 2 diabetes and, increasingly, for weight management — are among the most widely discussed drugs of this decade. Their cardiovascular and metabolic benefits are substantial and well-documented. What is less discussed, particularly for women, is their nuanced and sometimes contradictory relationship with sexual health and pelvic floor function. The research is still young, the picture is genuinely complicated, and that complexity deserves honest attention rather than either alarm or dismissal.
A Mixed Picture, Not a Simple One
The most important thing to understand about GLP-1 medications and sexual function is this: the evidence does not point in one clean direction. Some women report that their intimate lives improve after starting these medications. Others report the opposite. Both experiences are real, and both deserve to be taken seriously.
A 2025 women-only study found that 13.9% of participants reported decreased sexual function after starting a GLP-1 or GLP-1/GIP medication. That’s not a majority — but it’s not a negligible minority either. Roughly one in seven women in that sample noticed a change they didn’t want.
On the other side of the ledger, a 2025 survey of 2,000 single adults found that 52% said GLP-1 medications had affected their sex lives in some way. Within that group, 18% reported increased desire and 16% reported decreased desire; 16% said sexual function improved overall, while 12% said it worsened. These are survey-level numbers with all the limitations that entails — self-reported, not clinically verified, drawn from people who may have had strong feelings in one direction or another. But they reflect something real about the range of lived experience.
What explains such varied outcomes? A 2025 analysis noted that human findings on sexual function and GLP-1 medications are heterogeneous, with results likely shaped by drug type, metabolic status, and psychosocial factors. In other words: who you are, what medication you’re on, how long you’ve been on it, and what else is happening in your body and your life all appear to matter.
The Time Factor: Duration Changes the Numbers
One of the more striking findings from recent research is that the likelihood of reporting decreased sexual function appears to increase with longer duration of treatment. The 2025 women-only study found a clear time-dependent pattern: decreased sexual function was reported by 12.2% of those treated for six months or less, rising to 20.1% at seven months to one year, 22.3% at one to five years, and 30% at more than five years.
This matters for several reasons. First, it means that a woman who feels fine at month three may notice something different at month fourteen — and may not immediately connect that change to a medication she’s been taking comfortably for a while. Second, it raises questions (not yet answered by the literature) about whether these are cumulative pharmacological effects, whether they reflect changes in hormonal milieu over time, or whether other factors compound with prolonged use. Third, it underscores why ongoing conversations with a healthcare provider — not just an intake assessment — are important throughout the course of treatment.
What Might Be Happening Biologically
The honest answer is that researchers are still working this out. GLP-1 receptors are found not only in the pancreas and gut but in the brain, including regions involved in reward, motivation, and mood. This is part of why these medications are effective — they act centrally to reduce appetite and alter the reward value of food. But dopamine and norepinephrine, the same neurotransmitters involved in motivation and pleasure, also play significant roles in sexual desire and arousal.
Case-based literature published in 2025 describes instances of anorgasmia — difficulty or inability to reach orgasm — following GLP-1 initiation, with proposed mechanisms including altered blood flow and central neurotransmitter effects. These are not proven causal pathways in large populations; they are plausible hypotheses grounded in what we know about how these drugs act on the brain and vasculature. Pharmacovigilance data — the ongoing monitoring of adverse effects in people taking these medications — has identified reports of decreased libido and orgasmic dysfunction among GLP-1 users, but the statistical signal did not reach significance overall. That means we can’t say with confidence that the medication is causing these experiences at a population level — only that the experiences are being reported and deserve further investigation.
A 2025 pharmacology review also noted that while there is no large qualitative sex difference in overall therapeutic response to GLP-1 medications, there are quantitative sex differences, and possible interaction with estrogens. This is a significant area of unknowns: how these medications interact with the hormonal landscape specific to women — across the menstrual cycle, in perimenopause, in the context of hormonal contraception — is not yet well characterized.
The Pelvic Floor: What the Evidence Actually Supports
Conversations about GLP-1 medications and pelvic floor health tend to conflate two very different things: what is established and what is plausible. It’s worth being precise here.
What is established: weight loss, achieved through any means, can reduce the mechanical load on the pelvic floor. Obesity is a known risk factor for pelvic floor disorders including stress urinary incontinence — the kind where a cough, sneeze, or jump causes leakage — because excess intra-abdominal pressure chronically strains the connective tissues and muscles that support the bladder, uterus, and rectum. If GLP-1 medications help a woman achieve meaningful weight loss, that reduction in mechanical load is a genuine, indirect benefit to pelvic floor function.
What is not yet established: any direct effect of GLP-1 medications on pelvic floor muscles or connective tissue. Current research has not demonstrated a direct pelvic floor mechanism, for better or worse. This means that claims of GLP-1 medications directly strengthening or harming the pelvic floor are ahead of the evidence.
There is, however, one indirect pelvic floor concern worth knowing about: constipation. GLP-1 medications slow gastric motility — this is part of how they work — and constipation is a common side effect. Chronic straining during bowel movements is a recognized contributor to pelvic floor dysfunction, including prolapse and pelvic pain. This is not a reason to avoid the medications, but it is a reason to take the GI side effect profile seriously and address constipation proactively rather than waiting it out.
Two Myths Worth Correcting
Because these medications are so widely discussed — in doctors’ offices, in wellness spaces, in group chats — misconceptions accumulate quickly. Two in particular come up enough to be worth addressing directly.
Myth one: GLP-1 medications always improve your sex life because weight loss helps. Weight loss can improve body image, mobility, cardiovascular function, and hormonal balance — all of which can support sexual wellbeing. And for some women, that’s exactly what happens. But a meaningful proportion of women report worse sexual function after starting these medications, regardless of the weight loss. The body-confidence narrative, while sometimes true, is not the whole story.
Myth two: Sexual side effects from GLP-1 medications are well-documented and common. They are neither, yet. The evidence is mixed, the studies are limited in size and design, and population-level causality has not been established. This is an area of genuine scientific uncertainty — which means neither catastrophizing nor dismissing is warranted. What is warranted is attention, honesty with your provider, and ongoing monitoring.
What to Actually Do With This Information
If you are considering or currently taking a GLP-1 medication, sexual and pelvic floor health deserve a place in your conversations with your healthcare provider — even if bringing it up feels awkward, even if no one has asked. Research consistently shows that providers rarely initiate these conversations, and patients often don’t either, which means changes go unnoticed and unaddressed longer than they need to.
Here are evidence-informed approaches drawn from current clinical thinking:
- Establish a baseline before you begin. Note your current levels of desire, arousal, ease of orgasm, vaginal comfort, and any pelvic floor symptoms. This gives you something concrete to compare to later, rather than relying on memory of how things used to feel.
- Pay attention to timing. Changes in sexual function that appear shortly after starting the medication or after a dose increase are more likely to be medication-related than changes that develop gradually over an unrelated period. Tracking timing helps you and your provider think clearly about causation.
- Take constipation seriously. If you’re experiencing slowed digestion or straining, address it actively — fiber, hydration, movement, and conversation with your provider about a bowel regimen if needed. This protects your pelvic floor and your general comfort.
- Name specific symptoms. “Something feels off” is harder for a provider to respond to than “I’ve noticed I’m having more difficulty reaching orgasm” or “my interest in sex has dropped significantly since my dose increased.” Specificity leads to better care.
- Know that options exist. If sexual side effects are distressing, shared decision-making with your provider might include slower titration, switching to a different agent, or reassessing the overall risk-benefit picture. These medications are powerful tools — they are not mandatory at any cost.
- Hold complexity. You can be genuinely helped by a medication and also have legitimate concerns about one of its effects. Those two things don’t cancel each other out. Your sexual health is part of your overall health, and it deserves the same careful attention as your A1C or your blood pressure.
The Bigger Frame
Sexual wellbeing, as Emily Nagoski’s research has consistently shown, is not a fixed feature of a person — it is context-dependent, shaped by biology, stress, relationships, body image, and the stories we carry about ourselves. When a medication enters that system, it enters a complex one. Sometimes it helps. Sometimes it introduces friction. Often it does both at once.
What the research asks of us right now is intellectual honesty: we don’t have all the answers about how GLP-1 medications affect women’s sexual and pelvic floor health. The studies are recent, the populations studied are limited, and the mechanisms are still being mapped. What we do have is enough information to know that these effects are real for some women, that they may be dose- and duration-dependent, and that silence — on the part of patients and providers alike — makes them harder to address.
